Student Forms

Are you currently on a reduced price Membership agreement and need to either Suspend for a couple of months or Cancel? Here are the forms you need.

Print, Complete & Return to RhythmAddict. Forms must be filled out in their entirety. Any sections not completed and the form will be rejected and sent back to you for completion.

Cancellations require a full 30-day notice. The final payment date and amount must be present on the completed form, otherwise it will be rejected.

Suspensions require a 10-day notice. The next payment date and amount must be present on the completed form, otherwise it will be rejected.

Only ONE request per person. If you currently have your Membership on Suspension, and wish to Cancel, the Suspension is automatically canceled and the Cancellation request will take precedence. Since it is now just a Cancellation, the request requires the standard 30-day notice and final payment.
